The Department of Veterans Affairs Medical Center will be hosting an Enrollment and Information Event at the Riverhead Care Center for Nursing and Rehabilitation.
Guest speakers will provide overviews for three programs: Home Based Primary Care, Tele-Health, and MyHealtheVet.
Veterans and their families will be provided with information regarding eligibility, enrollment in healthcare, co-pays, and innovative programs and services. Hypertension screening and basic first-time physicals will also be provided by Nurse Practitioners.
Veterans should bring a copy of their discharge or separation papers (DD214). In order to complete the processing of the Application for Health Benefits (10-10EZ) the DD214 must be presented. If this document cannot be located, an Application for Military Records can be acquired at this event.
The VA Medical Center is committed to providing comprehensive health care services at our main Northport facility and at three clinic settings. The VA Clinic at Riverhead is conveniently located at the Suffolk County Center, Griffing Building, 300 Center Drive, Riverhead, NY.
It is a state-of-the art, refurbished setting equipped to offer primary care, mental health care and laboratory services Monday through Friday 8 a.m-4:30 p.m., with weekend and evening hours soon to be scheduled.
